PBH Provider Training Registration

Each class will cost $30.00 this will cover training manuals and materials for the class. Upon registering below, the provider will receive an email confirmation. This confirmation must be printed out and mailed with a Business Check, Certified Check or Money Order to:

No personal checks or cash will be accepted. Registrants can hand deliver any method of payments noted above to the Area Office at the location noted above. Please make sure the envelope is addressed to Rita Goodwin.

Upon PBH receiving your payment, your information will be entered into our database and you will receive a final email confirmation notifying you that your payment has been received and your space is guaranteed in the class. PBH must receive your payment 5 business days prior to the class. If the payment is not received you will be removed from the registration list and individuals on the waiting list will be invited to attend. No payment can be taken on the date of the training.

Should your check be returned for Non Sufficient Funds, you will be contacted and asked to pay for the class registration and the NSF Fee of $32 to PBH by Certified Check or Money Order.

Classes are limited to 20 individuals per class. Your space in the class is not guaranteed until payment has been received. No Refunds will be given.
